 To Register by Mail, or Fax, select your training course(s) and click on "Add To Cart," then "Check Out," and complete the registration form, print it, and fax it to us, with your credit card information, at 305-446-4978, or you may mail the completed form, together with your check payable to Mediation Services, Inc., 111 Majorca Ave., Coral Gables, FL 33134. For additional registration information, or special needs request, please contact our Training Coordinator at 866-401-4529. Mediation Services, Inc. (MSI) reserves the right to cancel any program, in which case a full refund of fees will be made. A $50.00 administrative fee will be charged for your cancellation or withdrawal until 14 days prior to any certified training. When you enroll in the course(s) we reserve a space for you, and therefore if you withdraw or cancel after 14 days prior to any training all payments will be deemed to be earned by MSI and no refunds will be issued.
